DOT Physicals Now Offered At CHI Memorial Convenient Care-Cleveland

CHI Memorial Convenient Care - Cleveland is now offering appointments for Department of Transportation physicals for those with a commercial driver’s license.  Tiffany Potter, APRN-BC, FNP-C, is certified to perform the medical exam.  

"The physical includes a health history review, a thorough physical exam and a urine test.  You will receive a completed Medical Examination Report for the Commercial Driver Fitness Determination form, as well as other supporting documents.
 You must pass all parts of the exam to be eligible to receive the CDL medical card," officials said.  "A DOT medical card with no restrictions is valid for two years.  A history of high blood pressure, taking high blood pressure medications, a sleep disorder, and taking oral medications for diabetes can restrict the medical card to one year or less, depending on the frequency your condition should be monitored."

The cost of the DOT physical is $75 and payment is due at the time of service.  DOT physicals are performed Monday through Friday and some weekends.  To schedule an appointment, or for more information, call 423-790-7750.  CHI Memorial Convenient Care - Cleveland is located in the Mouse Creek Publix Center at 645 Paul Huff Pkwy., Suite 105 in Cleveland.
