CHI Memorial Convenient Care - Cleveland offers appointments for Department of Transportation (DOT) physicals for those with a commercial driver’s license (CDL). 

Tiffany Potter, APRN-BC, FNP-C, is certified to perform the medical exam. 

The physical includes a health history review, a thorough physical exam and a urine test.  You will receive a completed Medical Examination Report for the Commercial Driver Fitness Determination form, as well as other supporting documents.  You must pass all parts of the exam to be eligible to receive the CDL medical card.

A DOT medical card with no restrictions is valid for two years.  A history of high blood pressure, taking high blood pressure medications, a sleep disorder, and taking oral medications for diabetes can restrict the medical card to one year or less, depending on the frequency your condition should be monitored.
