Jerry McIntosh, 65, of Cleveland, passed away at his residence on April 5, 2020.
Jerry was born to Charles and Edna McIntosh on Nov. 30, 1954 in Booneville, Ky.
He was a beloved dad, papaw and brother. He enjoyed time with his family, and anything law enforcement.
Jerry was a veteran of The U.S. Army National Guard, where he served for 20 years, and served 17 years as an officer of The Polk County Sheriffs Department and Benton City, where he held the office of police chief.
Jerry was preceded in death by his parents, Charles and Edna McIntosh and his brother-in-law, Earl Boston.
He is survived by his daughter, Tina (Mark) Ingram; son, Jason (Cheyenne) McIntosh; six grandchildren, Zayne McIntosh, Maddyson McIntosh, Jaxson Ingram, Charles Ingram, Landon McIntosh and Katie Jones; three sisters, Carolyn (Jim) Boone, Judy Boston and Debbie (Greg) Pennix; one brother, Bobby (Michelle) McIntosh; and several nieces, nephews and extended family and friends.
Companion funeral home is handling the cremation and his family will have a celebration of life at a later date.
In lieu of flowers, the family has asked for donations to be made to the Fraternal Order of Police or charity of your choice.
