Adam David Couch, 38, a resident of Chattanooga, Tennessee, passed away on Wednesday, February 28, 2017, at a local hospital.

Adam loved wrestling, music, Notre Dame Football, as well as sports in general. He was greatly loved and will be dearly missed by all who knew him.

He was preceded in death by his grandfather, George Kantorowski.

He is survived by his children, Gage and Izea; mother and step father, Georgann and Tom Perry; father, David Couch; grandmothers, Barbara Kantorowski, Catherine Couch; step-brother, Thomas Dean Perry II.

There will be a celebration of life memorial service on Saturday, March 10, at the Companion Funeral Home Cleveland Chapel at 3 p.m. with a visitation period prior to the service from 1-3 p.m. 

In lieu of flowers, the family requests that you make donations to the American Juvenile Diabetes Association.
