CHI Memorial Medical Group announces Ann Collins, DO, has joined CHI Memorial Family Practice Associates - Ooltewah.  

Dr. Collins earned a doctor of osteopathy from Edward Via College of Osteopathic Medicine at Auburn University in Auburn, Al.  She completed a family medicine residency at St. Vincent’s East Family Medicine Residency Program in Birmingham, and a hospice and palliative care fellowship at the University of Alabama at Birmingham.  She is board-certified by the American Board of Family Medicine.  Dr.
Collins also holds master’s and doctorate degrees in human performance from the University of Alabama in Tuscaloosa.

Dr. Collins joins William T. Bates, III, DO, Mark Crago, MD, PhD, Kent Grotefendt, MD. Bruce Johnson, MD, Allen Sherwood, MD, David Winters, DO, Alie Geren, NP-C, Cynthia Shaffer, NP-C, and Joel Smith, NP-C, at CHI Memorial Family Practice Associates - Ooltewah.  

The practice is located at CHI Memorial Ooltewah, 6401 Mountain View Road, Suite 109 in Ooltewah.  Clinic hours are Monday through Friday from 7:30 a.m.-7 p.m., and Saturday and Sunday from 8 a.m.-noon.  To schedule an appointment, call 423-495-5951.  Dr. Collins is accepting new patients.  Same day appointments and video visits are available.
